Are compression socks bad for your circulation?

Isn't constricting your leg actually bad for your circulation? This is an understandable concern, and a good question. In this article, we explain when compression is safe and supports circulation, when it can go wrong, and what signals indicate that you should take the sock off immediately.

The short answer

For a healthy user with a well-fitting sock, compression socks are not bad for circulation. In fact, gradual compression is precisely intended to support venous return. Justin, physiotherapist and co-founder of KINEX, explains: "Well-fitting, gradual compression is specifically designed to support venous return from the legs. For healthy users and the correct size, the sock should feel firm, without tingling, pain, or discoloration."

What the research says

A meta-analysis by O'Riordan and colleagues (2023, Sports Medicine), based on 22 studies in healthy and active participants, found a small positive effect on peripheral blood flow on average. The veins are supported, and the arterial supply of a healthy leg is not cut off by normal compression, simply because the blood pressure in arteries is much higher than the pressure a sock provides. The effect is small and varies by posture, so we don't promise better circulation for everyone, but it's not unsafe for healthy legs.

When can it constrict?

Problems mainly arise with the wrong size, a rolled-down cuff, or excessive pressure. This can lead to localized excessive pressure, especially around a narrow ankle or a bone edge. Justin: "Problems can arise if the size is incorrect, the cuff is rolled down, or the pressure is not appropriate. Then localized excessive pressure can occur." When to consult a doctor first?

There are medical situations in which you should not wear compression on your own. In certain forms of peripheral arterial disease, severe sensory disturbances, or diabetes with neuropathy, compression can threaten already limited blood flow (Rabe and colleagues, 2020, Phlebology). Justin is firm on this: in case of doubt, a doctor should assess first. This is not fine print; it's genuine advice.

Signals to take the sock off immediately

A good compression sock may feel firm, but it should not cause pain. Justin: "If you experience cold toes, numbness, pain, significant discoloration, or severe pressure marks, that's not a matter of just toughing it out. Take them off and get them assessed. Pain isn't always progress." So, pay attention to tingling, numbness, cold or discolored toes, and deep pressure marks. These are signals that the size or pressure is wrong, not that you should push through.

The conclusion

For healthy people, compression socks are not bad for circulation when they fit well, are worn correctly, and are suitable for the purpose. They may feel firm, but not painful, numbing, or constricting. In cases of vascular disease, diabetes with loss of sensation, or unexplained symptoms, a doctor should first assess whether compression is safe.
